Programme Dignity enables patients with advanced dementia to receive compassionate, specialised palliative care in the familiarity of their own homes. Through personalised care plans, our multidisciplinary team manages complex symptoms, reduces distress, and preserves comfort and dignity in the final stages of life. At the same time, caregivers are equipped with practical skills, emotional support, and guidance to better understand their loved one’s condition and respond confidently to challenging behaviours and care needs.

Programme IMPACT [Integrated Management and Palliative Care for the Terminally Ill Non-Cancer]
Programme IMPACT provides home-based palliative care to patients with end-stage organ failure, advanced lung disease, and frailty, which are conditions that are often unpredictable and emotionally demanding for families. By bringing medical, nursing, and psychosocial support directly into patients’ homes, the programme helps manage symptoms, reduce unnecessary hospital visits, and improve quality of life in their final months.

PROJECT DOVE [Dignity, Openness, Validation and Empowerment]
Project DOVE offers caregivers a continuous and integrated support journey, from the moment their loved one enters hospice care, through the end-of-life phase, and into bereavement. Our counsellors and medical social workers provide holistic psychosocial care, helping families process anticipatory grief, navigate difficult decisions, and cope with emotional and practical challenges along the way. After a loss, families continue to receive guidance and support as they rebuild their lives.

Make-It-Happen fulfils the final wishes of our patients, creating moments of joy, connection, and meaning at the end of life. As an Institution of a Public Character (IPC), your donation is eligible for a tax deduction in Singapore at the prevailing rate at the time of receipt (currently 2.5 times the donation amount).
