About Home Care

Supporting comfort and care at home

Many people living with a life-limiting illness prefer to remain in the familiarity and comfort of home. However, managing symptoms and daily care needs can become challenging over time. Our Home Care team provides medical, nursing and psychosocial care in your home, so that our patients receive professional support while staying close to their loved ones and the routines that matter.

With our Home Care service, patients will be supported by a multidisciplinary team focused on managing symptoms and supporting their wellbeing in a place that feels safe and familiar.

Who We Serve

We care for patients with advanced illnesses, including:

  • Patients with a life-limiting illness
  • Patients with cancer or non-cancer such as:
    • End-stage organ failure
    • Advanced dementia

Our team also supports patients and their loved ones in understanding the patient’s condition and care needs. We work closely with caregivers to provide guidance, education and support.

Patient Support

Multidisciplinary Care Team
To support our patients, our dedicated team of doctors, nurses, medical social workers and allied health professionals (including art and music therapists) provides comprehensive care encompassing:

  • Physical assessment and management
  • Emotional and psychosocial support
  • Spiritual care

Clinical Care
Scheduled home visits are arranged according to each patient’s needs, allowing us to closely monitor symptoms and ensure comfort. We provide regular follow-up and support in managing symptoms such as pain, breathlessness and agitation. The care plan is reviewed regularly and adjusted as needs evolve, ensuring appropriate support at every stage.

Support
A designated nurse-in-charge is assigned to ensure continuity of care and ease of communication. In addition, our team provides 24/7 on-call support for urgent medical advice, offering reassurance and peace of mind to our patient and their family at all times. 

Some of the other services we offer:

  1. Advance Care Planning
    We will guide the patient and their caregiver in discussing care preferences and making informed decisions for the future. 
  2. Caregiver Training
    We provide practical training for caregivers when needed, including:
    • Care of medical devices (e.g. indwelling catheters, nasogastric tubes)
    • Administration of medications
    • Safe patient transfers
  3. Equipment Support
    We provide recommendations and assist with equipment needs.
  4. Care Coordination
    We collaborate with hospitals, community partners and other healthcare providers to ensure continuity of patient care.
  5. Psychosocial and Allied Health Support
    Support from professionals such as medical social workers and therapists is provided as needed, as part of the patient’s overall care. Our medical social workers also support caregivers in managing the stresses of caregiving at home.

Teleconsultations
Teleconsultations are offered as part of our care service. These may be used in place of or alongside home visits, based on care team’s assessment, to enhance accessibility and enable timely review and follow-up.

Specialised Care Programmes

Programme Dignity
For Patients with Advanced Dementia
Launched in 2014 in collaboration with Tan Tock Seng Hospital, Programme Dignity provides specialised palliative care for patients with advanced dementia. As these patients may be unable to express their needs, the programme focuses on:

  • Individualised care plans
  • Comfort and quality of life at home
  • Equipping caregivers with skills and knowledge to manage distress

Programme IMPACT (Integrated Management and Palliative Care for Terminally-ill Non-Cancer)
Palliative Care for Terminally-Ill Non-Cancer Patients
Launched in 2020 with Tan Tock Seng Hospital, Programme IMPACT supports patients with non-cancer conditions such as end-stage organ failure and frailty.
The programme extends palliative care to patients whose needs are often under-recognised, allowing them to receive appropriate support at home.

Referrals and Admission

Patients should approach their primary doctor at the hospital or healthcare institution to submit a referral. The referring doctor will assess whether the patient meets the admission criteria. Our medical team will then review the referral and liaise with the primary doctor regarding the care plan.

Referrals are currently accepted from NHG hospitals only.

Once the referral is accepted, our team will contact the patient and caregiver to provide further information about the service and to make the necessary arrangements for the first visit.

Charges and Subsidies

Our home care services are provided at no charge to patients, who will not need to incur out-of-pocket payments for standard services. In certain situations, additional consumables or items may be required as part of care and can be claimed through MediSave, subject to prevailing policies.

Patients may also utilise MediSave from approved dependents (spouse, children, parents, grandchildren, and siblings).
